Blank Monster Entry

I continued on with making some blank documents and did a blank monster entry form for Full Metal Plate Mail.  It's pretty straightforward, so you just fill in the numbers. If a creature has only 1 special ability/attack, then just use the first entry and delete the rest.  It has a box for an illustration as well.  It's set up to mimic the basic layout of the FMPM manual but if somebody catches an error, please let me know.  I like making stuff like this so I suspect there will be more on the way.
