Immortality
Take the hearts of a mated pair of dragons (in campaigns where dragons are immortal) and brew them together with Black Lotus for 60 days, then soak the remainder in alcohol for three years. Then one person may drink under a full moon in one long swallow (you should have two quarts of liquid). If you fail to drink the whole thing without stopping there is no effect other than drunkenness. If you successfully drink the potion, then you sleep deeply for a month after which you become immortal versus old age, but not versus disease or injury.
Note that if dragonkind become aware of this, they will hunt you down. No, they will not kill you, but they will help you spend centuries hoping for death.
