Depending on which printing of the LBB's you have MR was introduced in the balrog entry. I have the 6th printing so I don't have that in my books, though someone on ODD74 did make a pdf of that page so I have the entry. MR is rated at a percentage and if successful will negate a spell entirely. This of course made it into later editions of the game.
How do you rule it in your games? do you use MR as written? or do you just give a bonus to saving throws vs spells? or base it off the dwarf -- save at 4 levels higher?

Hmm. Interesting.
My home game is young enough that this hasn't come up yet.
I have to say the dwarf idea is terribly enticing, being simple and representing magic resistance to me.
I'd probably go that route: about all I use percentile dice for anymore are random tables, as I prefer Delving Deeper's d6 resolution for such things as thief abilities, as opposed to the original percentage method in my WOTC reprints of the 6th OCE LBB.

I'm the same with Delving Deeper I tend to stick to d6 for everything when possible. It hasn't come up in my game yet either, but I used to do the percentile based. I'm thinking of just the saves myself instead of a flat it just doesn't work at all if the MR roll is made.

Yeah, I hear you, bro. And, it can still work like that if something is resistant only to a few types of magic or specific spells, too.
"The Grue is resistant to being charmed, and doesn't easily suffer illusions. It saves at four levels higher versus any illusions or charm spells."

Does anyone have access to see when this was introduced and how it reads?

Does anyone have stats for a Grue?
