What happens when a magical item breaks or fails? What happens to the magical energy stored, how hard are they to break? Is it somewhere between breaking the Staff of Power or the Staff of the Magi and nothing or is it something else entirely that happens.

My old Rolemaster Ref had them become unstable & eventually break causing an explosion if those caught in its blast & survived they got a magical mutation. The same occurred if you killed a Dragon & were soaked in its blood as it died, you then got a Dragon's gift (mutation) and other Dragons could sense a Drake Killer by the mystical taint. I am tempted to incorporate both into my future face-to-face campaigns.

IMC with the exception of final strikes from those two staves, magic items broken simply lose their charge and become inert. Of course, there are always exceptions.

I have thought about letting the maker of magical staves, rods and wands optionally building final strike capability into them. I have also thought about allowing line of sight remote detonation of a final strike by command word.

IMC Magic items don't break without a reason. They get lost, stolen, or leave the person who they no longer agree with or to go with someone who needs them more, but they don't break. If one does break, such as Anduril in LOTR, it was a spiritual blow to the entire bloodline. Anduril represented the True King, and was treated with reverence. To break an item which defines a character is to spiritually kill that character.
A powerful enemy can break an item, but when this happens it means something. I say we go on a quest to fix it!
