How do you handle religions, gods and goddesses in your campaign?

The third option is my choice.

As voted, above ... except no Neutral deities. Mother Nature, Gaia, etc.; represents Neutrality IMC, but she's more a primal force than definable deity.

3rd option, but I might eventually detail rites & holy day as the campaigns develop.

I'll do so after I finish my initial revisions. I will tease this - the world went through two God's Wars, one only a century past. The longed lived races survived it & remember it well. The world is still scarred by these events. Deities & their servants commune with the priests & oracles in their dreams; plus they occasionally walk among the people in but with only a mere fraction of their power.

in my game on the in game 1 day before it started all the gods died(at the hands of Lovecraftian gods. Very minor divine creatures stepped up to fill the void as this exhausted the Bad Guys. What the players never figured out was what the 6 or so gods did to survive was to travel everywhere together

I don't get too picky about it, they all stay off screen anyway.

I would go with number two. Most stuff gets hand waved.
