I beefed up overnight healing from rest. Because hit points are not so much physical damage, the recovery rate for rest/relaxation seemed a bit low. So, increased it according to something I read by Frank Mentzer (IIRC). He suggested [d6+(caster level)] as how CLW should work. I gave that to overnight rest, using the PC's level in place of caster level.
Nice thread topic, I'm looking forward to reading how others address this. Have an exalt.

I used to play to -10 hp, I still kind of like the idea. Binding wounds doesn't heal you, but it stops you from bleeding to death if you roll a saving throw. Players can attempt anything, if someone has herbs or something I may allow it and tells me how they are treating the injured person we can figure something out.

I've been debating back and forth on using death at zero HP or - Con = HP. Personally I like that PCs have a buffer before dying, it seems more heroic. If I use that rule, every round past if a PC once they hit zero HP, they bleed 1 HP a round till they get to negative Con/HP. So binding wounds can stop this bleed but if they take more wounds the bleeding restarts in addition to the wound damage taken. Note that if I use this house ruling, the wounded character is unconscious & unable to defend or move themselves.

I use binding wounds, plus I use healing salves and poultices and potions both magical and non-magical versions.

Binding wounds must always be done to stop the bleeding. The quicker it is able to be done, the faster you heal from the wound.

If damage exceeds 70% of hit points you will bleed out if you would are not bound up, if over 90% and no magical healing, you also have to cauterize the wound to prevent death.

Required to stabilize and stop the bleeding anytime magic healing is not used and often required to keep them alive until magical healing can be done.
